Are you interested in Volunteering with Sweet Dreams?
Volunteers are always welcome and are central to our operation. Sweet Dreams offers a variety of volunteering options from making cakes to participating with one of our Committees. No special talents are needed; only a passion in your heart and a desire to bring a little sunshine into someone's day. A great way to learn more about volunteer opportunities and meet current volunteers is to attend a Volunteer Meeting. Background Clearances
All volunteers are required to obtain a Pennsylvania Child Abuse History Clearance (Form CY-1113) and a Pennsylvania State Police Request for Criminal Record Check (form SP4-164). Volunteers should follow the steps below to obtain updated clearances.
 

1. Complete a Pennsylvania State Police Criminal Record Check (SP4-164) by visiting https://epatch.state.pa.us and following the directions to “Submit a New Record Check.” The ten dollar ($10) fee can be paid with a credit card online. To submit the Request for Record Check by mail with a certified money order, download a request form by clicking on the “Help” tab.

 

2. Once a volunteer has a response for the Criminal Record Check he can submit it along with the Pennsylvania Child Abuse History Clearance Form (CY-113). The form can be found at the following link:

B. The completed Child Abuse History Clearance Form and a ten dollar ($10) money order should be mailed to the address listed in the top left box on the form.
